Preparation of the Manuscript

1. General Instructions: The Journal encourages blind reviews. Any identifying author information on the manuscript should be limited to the title page. The only identifying information that should appear on each page is the page number and the title or running head of the manuscript.

Each component of the manuscript should begin on a separate page in the following sequence: title page, abstract and key words, text, acknowledgments, references, tables, legends for illustrations, and the illustrations Each line of the manuscript should be numbered in the left margin using the automatic line-numbering feature of the computer software. Line numbering will help the reviewers identify specific sections of the paper for comment to the author. Do not attempt to manually number the lines using tabs or spaces.

Authors must submit two hard copies of the original manuscript on standard 216 x 279 mm (81/2 x 11) white bond paper. One of the two hard copies of the manuscript should be blinded for peer review, with all author information removed. Type on one side of the paper only. Authors should keep copies of everything submitted.

Two sets of any tables, graphs, or figures should be included with the manuscript as high-quality laser prints. The shading or patterns chosen to define bar graphs or pie charts must be clearly visible and easily understood. Any photographs should be professionally prepared and submitted on glossy photographic paper.

In addition to the two hard copies, authors must include a diskette containing two electronic files of the original manuscript. One of the electronic files should be blinded for peer review, with all author identifying information and any acknowledgments or mentions of institutions or locations removed. The filename for the blinded electronic copy should contain the word "blinded." The other electronic file copy should include the entire manuscript containing complete author identifying information on the title page. The cover letter should be saved as a separate file named "cover letter." Software preferences is MSWord for document text and MSExcel for Tables. Tables must be saved as separate individual files on the disk with filenames Table 1, table 2, etc. All figures and photos must be saved as separate individual files on the disk with filenames Figure 1, Figure 2, etc. Charts and graphs may be submitted as MS Excel or MS PowerPoint files, or the original high-resolution prints may be scanned, saved, and submitted as .tiff files with a minimum resolution of 1200 dpi. Each electronic photograph must be saved in size 5 x 7 as a separate individual .tiff file with a minimum resolution of 1200 dpi. If the .tiff file is too large for an IBM-formatted disk, it may be submitted on a CD-R, CD-RW, DVD-R, or DVD-RW. All tables, charts, graphs, figures, and photos should be submitted in black and white unless color reproduction is necessary. If color figures are received, the author will be sent an estimate of the costs of color reproduction. If the authors approve the estimate for color reproduction, an invoice will be sent for payment before publication.

Preference is for the electronic version of the manuscript to include text, tables, and figures in separate electronic files using Microsoft Software. All diskettes submitted should be IBM-formatted high-density diskettes containing IBM-compatible files. The Journal cannot process Apple or Macintosh files or any electronic files that have been compressed or zipped. Check to make certain all electronic files are free of viruses.

The manuscript must be double-spaced throughout, including the title page, abstract and key words, text, acknowledgments, references, and legends for illustrations. Margins should be ample, with at least 1 inch of space on all sides. Pages should be numbered consecutively.

Use generic names of drugs, unless there is a specific trade name that is directly relevant.

Use only standard abbreviations as listed in the AMA Manual of Style, Eighth Edition. The full term for which an abbreviation stands should precede the abbreviation's first use in the text, except in the case of a standard unit of measurement. Avoid using abbreviations in the title and abstract. All manuscripts must be thoroughly edited for spelling and English grammar by the authors and/or an expert in English medical writing before submission. Any statistical analyses in the research or manuscript should be reviewed and verified for accuracy by the authors and/or a statistician before submission. Authors must include copies of permission to reproduce previously copyrighted material or illustrations.

2. Cover Letter: The editorial office must be sent a cover letter that designates one corresponding author and includes that author's complete mailing address, telephone number, fax number, and email address. The editorial office must be notified immediately if any author contact information changes. If possible, authors in medical school or residency training should also supply contact information of a mentor or additional author at the same institution. If the paper was part of a presentation to a professional association, this fact should be noted. If any of the authors have a conflict of interest, this should be explained in the cover letter.

3. Title Page: The title page should contain the title of the manuscript and the full name, highest academic degree, and affiliation of all the authors. Information about the authors should be listed on the title page in the order it is to appear in the publication. The name and address of the corresponding author and the name and address of the author to whom reprint requests should be directed should also appear. In the event an author changes institutional affiliation after submission but before publication, please provide both the institutional affiliation where the research was conducted, along with the current institutional affiliation of the author. If the paper was part of a presentation to a professional association, this fact should be noted. Any sources of support (e.g., grants, equipment), any disclosures or disclaimers, and a running head of not more than 40 characters (including spaces) belong on the title page. The title page should be a separate sheet, and it must be attached to the manuscript.

4. Abstract: A structured abstract is required for all manuscripts except for Commentaries, Clinical Notes, and Letters to the Editor. The abstract must be double-spaced. Abstracts for Research Articles should succinctly address, in 200 words or less, the following four categories: Objective, Design, Results, and Conclusions. Refer to current copies of the Journal for examples. Traditional one-paragraph abstracts are required for all other categories of papers including Brief Report, Case Report, Education & Administration, Literature Review, Analysis, and Perspective articles. Abstracts for Brief Reports and Case Reports should succinctly summarize, in 150 words or less, the salient elements and conclusion of the paper.

5. Key Words: Authors must include at least four "key words" (so labeled) on the line after the end of the abstract. Use appropriate subject headings as listed in the National Library of Medicine publication, Medical Subject Headings (1983).

6. References: Number references in the order in which they are mentioned in the text; do not alphabetize. Identify references with Arabic superscript numerals in the text, tables, and legends. References should be double-spaced, and the format should follow the current AMA style. Abbreviate the names of journals according to the format given in Index Medicus. References cited only in tables or figure legends should be numbered in accordance with a sequence established by the first identification of the particular table or figure in the text. Refer to current copies of the Journal for examples of the various types of references. All manuscripts except for extensive reviews of the literature should be limited to no more than 30 references. Authors may be asked to limit the number of references to conserve space.

7. Acknowledgments: All acknowledgments must be double- spaced and should appear on a separate sheet after the references. Acknowledgments should be removed from blinded copies for review.

8. Tables: Type tables double-spaced on separate sheets of 216 x 279 mm (81/2 x 11) paper. Title all tables and then number them in order of their citation in the text. If a table must be continued, repeat the title on a second sheet, followed by "Continued." Do not use any internal horizontal or vertical rules. See the Journal for examples. Camera-ready, computer-generated tables and graphs are acceptable. Software preferences for tables include Microsoft Word, Excel, and PowerPoint.

9. Illustrations: Four sets of illustrations must be submitted. Photographs must be black and white glossy prints 127 x 173 mm (5 x 7). The Journal encourages the submission of color photographs and figures for publication, but the author assumes responsibility for the cost of color production. If color photographs and figures are submitted, the Journal will provide the author with an estimate of the color production costs. Legends are required for each illustration and must be typewritten and double-spaced on separate sheets of standard-sized paper. Each legend should contain a title, and both illustrations and legends should be numbered according to the order of their citation in the text. Tables and figures should be submitted blind. Affix a label to the back of each figure, giving the figure number, the article title, and an arrow indicating "top." Do not write directly on the figure itself. Do not identify the author on the back of tables or figures. If photographs of persons are used, written permission from the patient must accompany the submission.

10. Measurement Units: Measurements of length, height, weight, and volume should be reported in metric units. Temperatures should be written in degrees Celsius. Blood pressures should be given in millimeters of mercury. All hematologic and clinical chemistry measurement should be reported in the metric system in the terms of the International System of Units (SI).

11. Statistics: Describe statistical methods with enough detail to enable the knowledgeable reader with access to the original data to verify the reported results. When possible, quantify research findings with appropriate indicators of measurement error or uncertainty (such as confidence). Avoid sole reliance on statistical hypothesis testing, such as the use of P values, which fails to convey important quantitative information. Discuss eligibility of experimental subjects. Give details about randomization. Describe the methods for, and success of, any blinding of observations. Report treatment complications. Give specific numbers of observations. Report any losses to observation (such as dropout from a clinical trial). References for study design and statistical methods should be to standard works (with pages stated) when possible rather than to papers in which designs or methods were originally reported. Specify any computer programs used.

12. Ethics: When reporting experiments on human subjects, indicate whether the procedures followed were in accordance with the ethical standards of the responsible committee on human experimentation (institutional or regional) or with the Helsinki Declaration of 1975, as revised in 1983. Do not use patients' names, initials, or hospital numbers in text or legends. Indicate whether the institution's or the National Research Council's guidelines for, or any national laws on, the care and use of laboratory animals were followed.

Review Process

Receipt of a new manuscript hard copy and diskette will be acknowledged via email. The corresponding author will be informed of the manuscript ID number and requested to reply via email with any additional information necessary to facilitate the review process. After all the required author information, manuscript hard copies, and electronic files have been received, an initial review will take place to determine if the manuscript is appropriate for the Journal. Following the initial review, the author will be notified by email whether the manuscript has been selected for the extended peer review process. If the paper has been selected for extended peer review, the copyright release will be attached to the author email notification. The copyright release must be completed and returned to the Journal editorial office. Following the extended peer review process, authors will be notified of the editorial decision by email. Authors may be asked to revise the manuscript according to the reviewer's comments and to return hard copies and electronic copies of the revised manuscript.

Revising a Manuscript

Authors should use the tools/track changes feature in Microsoft Word to edit the paper. Show all additions to the text in red underline and all deletions in red strikeout. Send to the Journal two electronic files: one file should show all editing changes in the text, and the other file should be a final cleaned-up version after all changes have been accepted. Send the two manuscript files (marked and clean) electronically as email attachments. Also, send a 3 1/2-inch diskette along with two hard copies of the revised manuscript. Email attachments and the diskette should both contain the two revised manuscript files (marked and clean), plus the cover letter explaining each change. One of the two hard copies should show the additions and deletions, and the other copy should be clean with all changes accepted. The filenames should indicate the manuscript ID number, plus "revised marked" or "revised clean." If no additional changes are requested, the author will receive an acceptance notification by email.

Publication

The goal of the Journal is to publish all accepted manuscripts as quickly as possible. Accepted manuscripts are generally published in the order received. Authors will receive page proofs via email approximately three months before the month of publication. This will be the final opportunity for authors to proofread the article before publication. Instructions on how to order author reprints will be included with the page proofs.

Continuing Medical Education

The Journal considers quality research articles to be published and highlighted as a continuing medical education activity. Manuscripts selected to be CME articles are sometimes published more quickly than other papers. Authors who wish to have a manuscript considered as a CME article should include the following items with the original manuscript submission:
  • Three objectives answering the question: "Upon completion of this article, the reader should be able to:"
  • Five questions/answers for self-assessment
  • A sentence about each author that describes institution affiliation and current position.
Including the above with the original manuscript submission will greatly expedite consideration of the manuscript for publication as a CME article. See examples in the Journal.

Letters to the Editor

The Journal welcomes intellectual and scholarly letters of comment about articles published in the Journal or other matters of general interest. Letters should be typed on standard paper, double-spaced with 1-inch margins. References may be included to support opinions. The Editor reserves the right to determine which letters shall be published.

Commentaries

The Journal encourages authors to contribute expert opinion in the form of a short commentary. A commentary is a concise paper that promotes a particular viewpoint. Papers that follow the format for a commentary do not have abstracts and should be limited to no more than eight double-spaced typed pages including references. Include four key words. Authors should submit four hard copies plus a diskette.

Literature Reviews

The Journal considers literature reviews submitted by experts on a particular topic of interest to the readers. Because of space considerations, literature reviews will be selected for peer review only after careful evaluation of content and author expertise.

Poster Abstracts

Abstracts of scientific posters presented at the Annual AAP Spring Meeting may be published in the Journal. Although there is no peer review, the Editor may shorten or redact abstracts.

Authorship

The American Journal of Physical Medicine & Rehabilitation accepts the guidelines for authorship published in the Uniform Requirements for Manuscripts Submitted to Biomedical Journals. Persons designated as authors must meet all of the following criteria: (1) contributing to the conception and design or analyzing and interpreting data; (2) drafting the article or revising it critically for important intellectual content; and (3) approving the final version to be published. Supporting the study or collecting data does not constitute authorship. Authorship based solely on position (e.g., research supervisor, department head) is not permitted.

Corporate Sponsorship and Conflicts of Interest

The editors of the American Journal of Physical Medicine & Rehabilitation are extremely concerned about the appropriate disclosure of any real or perceived conflicts of interest. Authors must define any and all funding sources supporting the submitted work. All corporate sponsors must be identified, even if their support is indirect, e.g., to a local research foundation that funded the project. The authors must disclose any commercial associations that might pose a conflict of interest in connection with the work submitted for publication. Other associations such as consultancies, equity interests, or patent-licensing arrangements should be noted at the time of submission. All disclosure information should be included on the title page of the manuscript.

Appeals Process

Appeals must be made in writing within 1 month of receiving the decision regarding a manuscript. A previously rejected manuscript may be resubmitted with a cover letter explaining why the decision is being appealed. The cover letter should also explain any changes that have been made in the manuscript. The author will be notified of the final decision.
