Instructions to Authors

All manuscripts and correspondence, not previously published, simultaneously submitted, or already accepted for publication elsewhere, are to be submitted to the Editor: if possible, please submit online (http://cephalalgia.manuscriptcentral.com), but if this is not possible, submit your manuscript by post (as paper copies with a disk).

Online submission ensures the quickest possible review and allows authors to track the progress of their papers online. Authors are encouraged to supply the names of potential referees and/or referees they do not wish to review their paper. Manuscripts will be handled by the Editor and Associate Editors and authors are asked to supply the name of the appropriate person.

Do not duplicate your submission by submitting online and by post.  Where possible please use online submission.

PREPARATION OF MANUSCRIPTS

All submissions should be clearly written in English. The text should normally be divided into Instructions, Methods, Results, Discussion, and References. No specific format is required for Letters to the Editor.

Use standard abbreviations. Any non-standard abbreviations (to be avoided if possible) should be explained in the text the first time they are used. Avoid abbreviations in the title. Quantities and units should be expressed in accordance with the recommendations of the International System of Units (Syst¨¨me International d'Unit¨¦s).

Reports of clinical trials should adhere to the tenets of the CONSORT statement (JAMA 2001; 285: 1987-1991).  A flow chart should be provided describing the progress of patients through the trial.  A checklist of CONSORT requirements must also be completed. Both forms should be sent to the Editorial Office by post or fax. Alternatively, scanned copies of the hand-signed forms can be e-mailed.

Title Page
The title page shall contain: (1) A short, concise but informative title (2) The full names of the author(s) and the respective affiliations. (3) Name and full address of the author to whom galley proofs and reprint requests should be sent. Include a telephone number, a fax number and an e-mail address.

Abstract
An abstract not exceeding 150 words should be presented on the second page. The abstract should state briefly the objective of the investigation, basic procedures, main findings, and principal conclusions. List no more than five key words.

References
References should be included in the file for upload or, for postal submission, be typed double-spaced on a separate sheet(s), numbered consecutively in the order in which they appear in the text (Vancouver style). Accuracy of the reference list must be verified before submission of the manuscript. Identify references in the text, tables and legends by arabic numerals in parentheses. For papers with up to seven authors, the names of all authors should be listed. For papers with eight or more authors, the first six names should be listed, followed by et al. The titles of journals should be abbreviated in accordance with the style of the Index Medicus. Use the following format for journal articles and book references.

Book Chapter
1. Kaada B. Neurophysiology and acupuncture: a review. In: Bonica JJ, Albe-Fessard D, editors. Advances in pain research and therapy. Vol 1. New York: Raven Press, 1976:733-41.

Journal Article (less than seven authors)
2. Dahlof CGH, Dimenas E. Migraine patients experience poorer subjective well being/quality of life even between attacks. Cephalalgia 1995; 15:31-6

Journal Article (more than seven authors)
3. Trillat AC, Malagie I, Scearce K, Pons D, Anmella MC, Jacquot C, et al. Regulation of serotonin release in the frontal cortex and ventricular hippocampus of homozygous mice lacking 5-HT1B receptors: in vivo microdialysis studies. J Neurochem 1997;69:2019-25

Journal Supplement Article
4. Headache Classification Committee of the International Headache Society. Classification and diagnostic criteria for headache disorders, cranial neuralgia, and facial pain. Cephalalgia 1988;8 Suppl 7:1-96

References to material available on the World Wide Web can be given, but only if the information is available without charge to readers on an official site. Authors will be asked to provide electronic copies of the cited material for inclusion on the Cephalalgia web page at the discretion of the Editor. The format for citations is as follows.

Beckleheimer, J. (1994). How do you cite URLs in bibliography? [WWW document]. URL http://www.nrlssc.navy.mil/meta/bibliography.html

Tables
Include tables in the file for upload. Use the table function of a word processor rather than tab and space. For postal submission type each table, double-spaced on a separate page. Do not submit tables as photographs. Number tables consecutively with arabic numerals and give each a short, descriptive heading. Give each column a short or abbreviated heading. Place explanatory matter in footnotes to the table, not in the heading. Explain in the footnotes all non-standard abbreviations used in the table. Omit internal horizontal and vertical lines. If data from another published or unpublished source are used, obtain permission and acknowledge fully. Tables should be self-explanatory and understandable without reference to the text of the article.

Figure Legends
Each figure should have a legend containing sufficient information to make the figure intelligible without reference to the text. Figure legends should be included in the file for upload. For postal submission, all the legends must be typed double-spaced on a separate page(s). If a figure has been published previously, acknowledge the original source and submit written permission from the copyright holder to reproduce it.

Figures
If submission of digital illustrations is not possible (see section below) then to ensure clear reproduction, it is preferred that illustrations are submitted as 11 x 17cm (5 x 7 inch) glossy prints unmounted and untrimmed. Prepare drawings and graphs with black India ink on white background, using no typewriting or computer print. Letters, numbers and symbols should be clear and even throughout and large enough to be legible when reduced for publication. Do not attempt to hand-letter line drawings and photographs.

If submitting by post, all figures must be submitted in triplicate and have a label pasted on the back indicating the figure number, the top of the figure, and the first author's name. Do not write directly on the back of the photographs or damage them by using paper clips. If photographs of persons are used, either the subjects must not be identifiable or written permission to use the photographs must accompany the manuscript.  It is the policy of Cephalagia for authors to pay the full cost for the reproduction of their colour artwork. If colour figures are requested, Blackwell Publishing require you to complete and return a colour work agreement form before your paper can be published.  This form can be downloaded as a PDF from the following web address: http://www.blackwellpublishing.com/pdf/Sub3000_X_CoW.pdf.  If you are unable to download the form, please contact the editorial office.

The Editor will not return manuscripts. If the author(s) wishes to have the figure(s) from a rejected article returned, a self-addressed, stamped envelope must be provided.

ON ACCEPTANCE: DISK SUBMISSION

On acceptance a disk and hard copy of the manuscript must be provided. Accepted manuscripts will be published from the word processor file (not PDF) on disk wherever practicable and therefore must be provided in this form. This reduces typesetting errors and speeds up publication. Authors must ensure that all sections of their paper, excluding figures, are presented in a single file, updated to incorporate all revisions, and that the version on the disk matches the final hard copy. The acceptance date for the manuscript will be the date at which the correctly formatted disk arrives at the production office. All accepted manuscripts must be accompanied by a signed copyright assignment form.

Text
Files should be formatted double-spaced with no hyphenation and automatic word-wrap (no hard returns within paragraphs). Please type your text consistently, e.g. take care to distinguish between '1' (one) and 'l' (lower-case L) and '0' (zero) and 'O' (capital O), etc.

Tables
Tables should be created using the table editor function of word processing software. Do not use graphics software to create tables.

Mathematics
In-line equations should be typed as text were possible. Graphics programs and 'equation editors' can also be used, and the output will be used were possible but displayed equations may occasionally be rekeyed by out typesetters.

Figures
On acceptance of a paper, please send us digital versions of your figures if at all possible. EPS and TIFF files are preferable for print reproduction. Please note that your paper will go through production more quickly if the figures do not have to be relabelled or redrawn.
Please ensure that electronic artwork is prepared such that, after reduction to fit across one or two columns or two-thirds width (80mm, 169mm or 110mm respectively) as required, all lettering will be clear and easy to read, i.e. no labels should be too large or too small. Avoid using tints if possible; if they are essential to the understanding of the figure, try to make them coarse. Always enclose hard copies of digitally supplied figures. No artwork for publication should be incorporated into the final accepted text files. Original drawings or photographs should be supplied for reproduction.
Full details of submission of manuscripts and artwork are available at http://www.blackwellpublishing.com/authors/prep_illust.asp.

PROOFS

After the manuscript has been prepared for publication, the proofs will be sent via e-mail as an Acrobat PDF (portable document format) file. The e-mail server must be able to accept attachments up to 4 MB in size. Acrobat Reader will be required in order to read this file. This software can be downloaded (free of charge) from http://www.adobe.com/products/acrobat/readstep2.html. This will enable the file to be opened, read on screen, and printed out in order for any corrections to be added. Further instructions will be sent with the proof. Proofs will be posted if no e-mail address is available; author who may be absent should arrange for a colleague to access their e-mail to retrieve the proofs.

Proofs should be corrected and returned immediately by express mail. Printer's errors should be marked in red, and other alterations in blue. Authors will be liable to pay for any alterations not the result of printer's errors which they make. A form for ordering offprints and a price list will be provided with the author's proofs. Twenty five offprints will be supplied free of charge.

COPYRIGHT INFORMATION

A Copyright Assignment Form will be forwarded by the editors. The senior author must sign the form, acting on behalf of all authors. Until this form is received by the editorial office the manuscript will not be put into production.
